Your role at PFH:

As a Remote Support Staff member, you are responsible for the daily monitoring of individualizes with I/DD, aging, etc. You will:

  • Continue monitoring of dashboard of assigned clients during service hours.
  • Following protocols and parameters for sending notifications for each client to designated contacts.
  • Reviewing of Assessments, ISP’s, and Parameter Sheets for all assigned clients. Must be knowledgeable and aware of all information in the client files.
  • Document any notifications and the response given for that notification. Documentation will include great detail and meet Medicaid adequate documentation guidelines.
  • Notify to all designated contacts of issues via email, text or phone calls of all issues during service hours.

We think you’d be great for this role if you have:

  • Must be 18 years of age; must have a high school diploma or GED.
  • Experience and basic knowledge with the DD field and population is preferred.
